2019-20 Frozen Fins

Thank you for registering for the Fox Mill Woods 2019-20 Frozen Fins season.

During the registration process, you will be asked for your name, address and swimmers information. If you were a member of the 2019 FMW Swim Team, much of the information should pre-fill if you have logged into your account. If you are not already logged in, please use the "Returning member? login" button above to do so.

While we are opening registration to all interested parties, this registration process will not ensure your swimmers a spot in our program. For example, the program does not require FMWS&TC membership, however we will give priority to club members. In addition, we have a fixed amount of lane space that cannot be exceeded. Once the maximum enrollment is reached, we will keep record of a waiting list. We will notify you no later than Sunday August 25th of the status of your enrollment; in most cases we will notify you sooner. If your enrollment is confirmed, payment by check will be required at that time.

Please answer the form questions to help us determine your swimmers status and lane assignments. If you have multiple swimmers with different answers, please use the "other" option and provide a brief answer.

Assumption of Risk and Release Agreement

As the parent of a member of the Fox Mill Woods Frozen Fins program, I assume full responsibility for any injuries, damages, or losses which may occur to the swimmer or swimmer's property and hereby release Fox Mill Woods Swim Club and its officers, members, agents and any persons assisting in its activities and functions from any claims present or future, whether known, anticipated or unanticipated, resulting from or arising out of any swimmer's participation in swim team activities.

I hereby waive and release any and all claims that I may have against any Coach of Fox Mill Woods Swim and Tennis Club, Inc. and any Team Representative of Fox Mill Woods Swim and Tennis Club, Inc. or any members of the Board of Directors of Fox Mill Woods Swim and Tennis Club, Inc. acting on behalf of my child with regards to any accident, injury, or sickness that may occurr while my child is under the direction and control of the Fox Mill Woods Frozen Fins program.

Use of Photos

I authorize FMWST the right to take photographs of me and my family in connection with Frozen Fin events. I authorize FMSWT, its assignees and transferees to copyright, use and publish the same in print and/or electronically.
